Have you enabled devtmpfs in the kernel?

Device Drivers  --->
   Generic Driver Options  --->
     [ ] Support for uevent helper [CONFIG_UEVENT_HELPER]
     [*] Maintain a devtmpfs filesystem to mount at /dev [CONFIG_DEVTMPFS]
