passwd command

I recently booted up an old LFS system that hadn't been used in over a
year. (So I'm not sure what may have changed to cause this trouble.)

There isn't really a problem so much as an annoyance.
When changing a password, I am prompted 4 times.
The interaction looks something like the following...

root:~# passwd
New Linux password:
Retype new Linux password:
Enter new UNIX password:
Retype new UNIX password:
root:~#

Any ideas what may be causing this behavior?

base system: LFS-6.3
shadow-4.0.18.1
Linux-PAM-0.99.10.0
